This health assessment provides information to allow you to be proactive about your health and make lifestyle changes to reduce any risk of future problems. It is also advisable for patients with chronic disease such as Type 2 Diabetes, Asthma, Chronic Obstructive Pulmonary Disease (COPD), Cardiovascular Disease (heart failure, angina, stroke, irregular heartbeat) to have an annual health check.

In the first instance you will meet with one of our nursing team who will complete a set of blood tests with an ECG and BP Monitor where appropriate.

 

Once your results are back, you will have a consultation with your GP to discuss your lifestyle, adjust your medication and formulate a plan to improve your health.
